YOUR FIRST VISIT:
New clients will need to fill out this intake form. Dr. Iverson will review this and then contact you to register you and/or schedule your Initial potentially suggest a more appropriate referral for you. You will then need to register with the Simple Practice Patient Portal (electronic medical records program). Now you can move on to viewing and requesting visits.

Returning clients who have either not received treatment in the past 6 months or have a new injury will need to contact the clinic to have their Simple Practice Patient Portal re-activated. Please try logging into your portal link first as it may still be active: https://shinept.clientsecure.me

VIEWING AND REQUESTING VISITS:
All registered clients are registered with Simple Practice Patient Portal which can be accessed via their website https://shinept.clientsecure.me or via the Simple Practice App. The website will always make you login again with an email so you may prefer the app.

Once there, please click on Request Appointment to view the available appointment times. You can request the time(s) you would like and that request will be confirmed as soon as possible, generally within 2 business days. If it is not accepted, we will share why.

You will definitely find that consistency is best! To that end, we strongly recommended scheduling weekly Follow-Up Visits after your Initial Evaluation. Every-other-week visits can also work. We offer teleheatlh (virtual) visits, which can help you maintain the consistency of visits. Without evaluating you first, it is challenging to recommend how many visits anyone will need, but scheduling at least 6 Follow-Up visits out after your Initial Evaluation is best. If you end up not needing them, or need to reschedule, that is never an issue. It is better to have them if you don’t need them than the other way around.

CANCELING:
You are able to cancel upcoming visits on the Simple Practice Patient Portal. Please note that cancellations made within 24 hours of your appointment will be charged at $100 for first missed session and full amount for all subsequent missed sessions. Late cancellation policy is full removal of all clients upcoming visits if two cancellations (in less than 24 hours of the visit) occur.
